New Patient Registration |
When arriving at the Health Centre, please collect a Registration Pack from Reception for each member of the household. You will need to complete the Registration Forms and return them to the Health Centre together with the documents shown on the form.
When you have supplied your details you will be asked to make an appointment with one of the nursing team to have a New Patient Check.
MEDICAL CARD:
If you are new to the NHS, your new Medical Card will take between 2 and 3 weeks to arrive from the Health Authority.
All new patients wishing to Register with the Lakeside Medical Practice MUST complete the GP Questionnaire. (Please fill in your details that are relevant and tick the relevant boxes). You MUST PRINT this Form before completion.

Forms MUST be printed off before completion and taken to the Health Centre in person together with any documents requested on the forms.
Each family member must have a set of completed forms.
